Everybody knows that adding forms to a web site is a pia...
In my opinion (which sometimes doesn't mean that much) the following web site is the best for forms..and it's FREE...just put a link to their site on your site.
It's fast & easy.

Good link to save, but only one form allowed per account and its advertising supported.

I used this on a friend's website where their free hosting did not support PHP scripts. Seems OK but you may have to do a little HTML tweaking to get things looking exactly the way you want. Not bad for a free service.
There is a simple forms editor, and the form contents gets emailed to

I stuffed the code into an image object with a visible border, and explicitly colored all the fields, as the "required" ones weren't appearing on all browsers correctly. I also added some empty table rows to give the layout a little more space. (Is there a better way to stretch tables?)
Please don't be too hard on me as I'm only a beginner!:confused:

I implemented it per your instructions and everything seems ok, except I don't receive the email w/ the info. from the form. Does the email address you recieve the info. at have to be on the same server/domain name as the server/domain name the form is hosted on?

Your server has to support PHP for the form to work. I have a new sideline biz I'm tryin to work doing forms, actually just processing the forms. Try the form and code at the form site listed below in my sig.
